Need a DOT physical exam? Well, look no further. You've come to the right place!

All you need to bring with you to your appointment is your driver's license and a list of any medications you are taking. We will provide you with a medical card and/or long form once you have successfully completed the DOT physical exam. From there you will need to provide your medical certificate to your local DMV in order to obtain or reinstate your Commercial Driver's License (CDL). Upon completion of your DOT physical we will submit your examination results directly to the Department of Transportation.
